Most “best medical billing companies” lists are written by a billing company that puts itself first. This one is written by a billing company too, so here is the deal: BilNow is on the list, every figure comes from the company’s own public pages or Semrush data as of September 2026, and where a company does not publish a price we say so rather than guess. Use the table to shortlist, then check the two questions at the end before you sign anything. Last reviewed September 2026.

What the table shows
Only one company on this list publishes its rate. That is not a criticism of the others; enterprise vendors price by contract. But for a private practice comparing quotes, it means every other number arrives after a sales call. Industry surveys put outsourced billing between 4% and 9% of collections; anything you are quoted should be read against that range and against the collection rate the vendor commits to. BilNow’s rate for private practice billing services is 2.7% of collections, with no setup fee.
Half the list requires their EHR. CareCloud, AdvancedMD, athenahealth and Tebra bill best, or only, on their own platforms. If you are happy with your current EHR, that narrows the field to vendors who work inside it: BilNow, Medusind, BillingParadise, and GeBBS for hospitals.
Scale cuts both ways. Medusind and GeBBS can absorb a 40-location group; a solo behavioral health practice will be a small account there. BilNow is the smallest company on this list and that is the honest trade-off: a 1,200-provider client base means a two-provider practice is a meaningful account, and it also means fewer public reviews than a vendor with 6,000 providers.
Two questions that decide it
- What is your net collection rate and first-pass acceptance, in writing? A biller at 4% collecting 92% of what is collectable costs more than one at 5% collecting 97%. BilNow publishes 98% first-pass; ask every vendor for theirs and for a clearinghouse report to back it.
- What happens to my data and my EHR if I leave? Percentage-of-collections contracts should have no termination fee and leave your EHR, your patient data and your payer enrollments exactly where they were. If a vendor’s answer involves migrating you to their platform, price the migration and the exit before the billing rate.
