FLORIDA AT A GLANCE
QUICK ANSWER
BilNow is a New York-registered medical billing company serving practices across Florida, providing end-to-end revenue cycle management to Florida practices: patient registration and eligibility, medical coding, claim scrubbing and submission, denial management, A/R recovery, payment posting, patient billing, prior authorization and credentialing. Workflows are built for Florida’s Statewide Medicaid Managed Care program, PIP/no-fault claims under Fla. Stat. § 627.736, Florida Blue, workers’ compensation under the Department of Financial Services, and Medicare Advantage. Pricing starts at 2.7% of collections with no setup fee.
KEY TAKEAWAYS
- SMMC runs across 11 regions; the MCO roster and the rules change by region.
- PIP is primary for auto injuries and requires initial treatment within 14 days of the accident.
- Florida requires billing agents and clearinghouses to be enrolled and linked to the provider — ask any vendor to prove it.
- Medicaid provider agreements must be renewed inside a 90-day window, or claims suspend without retroactive payment.

Why Florida Practices Need a Smarter Billing Strategy
Four payer systems, four sets of rules — on the same patient panel.
Florida practices may work with Medicare, Florida Medicaid and its managed-care plans, commercial insurers, workers’ compensation carriers and self-funded plans — each with different requirements for eligibility, authorization, claims, documentation and follow-up. Medicaid runs almost entirely through the Statewide Medicaid Managed Care program, administered by AHCA across 11 regions; fee-for-service is rare. Add a no-fault auto system with its own fee schedule, an independent Blue plan, and Medicare Advantage enrollment above 60 percent, and a one-size-fits-all billing process does not survive contact with a Florida payer mix.

SMMC and the region problem
The MCOs available — Sunshine Health, Simply Healthcare, Humana Healthy Horizons, Molina, Aetna Better Health, UnitedHealthcare Community Plan — vary by region, and each carries its own prior-auth rules, encounter formats and filing windows. Contracted providers typically have 180 days to file. BilNow routes claims by the member’s plan and region, not by a single Medicaid payer ID.
Your billing agent must be enrolled too
This is the Florida rule most practices never hear about. Under AHCA’s provider enrollment policy, billing agents and clearinghouses that submit transactions on a provider’s behalf must themselves be enrolled and linked to the provider group, with an EDI agreement in place. A billing company that isn’t properly linked can stop your Medicaid claims from moving at all. Ask any Florida billing vendor to show you their enrollment.
The 14-day PIP rule (Fla. Stat. § 627.736)
Florida is a no-fault state. For motor-vehicle injuries, Personal Injury Protection is the primary payer, pays 80 percent of covered medical expenses regardless of fault, and requires initial treatment within 14 days of the accident to qualify. PIP runs on a separate fee schedule with shorter billing windows. BilNow flags PIP-eligible encounters at registration so both clocks are tracked from day one.
Florida Blue is not Anthem
Florida Blue is an independent licensee owned by GuideWell, headquartered in Jacksonville, with its own submission windows (365 days from date of service), appeal deadlines and EDI workflows. Applying another Blue plan’s rules to Florida Blue creates avoidable rework.
Medicare Advantage above 60 percent
More than 60 percent of Florida’s Medicare beneficiaries are enrolled in MA plans, each with its own prior-authorization and appeal process. BilNow keeps MA workflows separate from Original Medicare and Medicaid so authorization gaps are caught before submission.
Snowbirds and out-of-state coverage
Every winter, Florida practices see patients whose primary coverage lives in another state — an out-of-state Medicare Advantage plan with its own network rules, a home-state Blue plan billed through BlueCard, or a commercial plan with no Florida contract. Coverage that was valid in October can be out-of-network in Florida in January. BilNow verifies out-of-state plans at registration, routes BlueCard claims through the home plan correctly, and flags MA members whose plan has no Florida network before the visit — not after the denial.

WHAT CHANGED IN 2026
Two things Florida practices should re-check this year. First, following the current SMMC contracts and AmeriHealth Caritas’ exit, any patient previously assigned to a departed plan needs their current MCO re-verified before billing — those denials cannot be appealed on timely-filing grounds. Second, Medicaid provider agreements expire on a rolling basis and the renewal link only appears in your portal during the 90-day window before expiration; miss it and claims suspend until the new agreement is approved, generally with no retroactive reimbursement.
Your Practice Deserves More Than a Claim Submission Service
You need a revenue cycle partner.
Submitting a claim is only one step in getting paid. A claim can be coded incorrectly. A payer can reject it. An authorization can be missing. A payment can come in lower than expected. A denial can sit unresolved while an account ages for months. In Florida, it can also be sent to the right plan in the wrong region, or miss a PIP window that closed on day 15. Every unresolved issue affects your cash flow.
BilNow manages the entire revenue cycle — not just the claim. Registration → eligibility → coding → claims → payment → denials → A/R → revenue. One team, one connected workflow, one objective.

Complete Medical Billing Services in Florida
Everything your revenue cycle needs.
Nine services delivered by experienced billers and coders working inside your existing EHR, with SMMC regions, PIP rules, Florida Blue, workers’ comp and MA workflows built in.
Many billing problems start before the patient is seen. We verify demographics, coverage, benefits and authorization requirements up front — including SMMC region and plan, PIP eligibility for auto injuries, and MA vs. Original Medicare status — so the claim is aimed correctly from the start.
Your providers deliver the care; our experienced coders make sure it is represented accurately. ICD-10-CM, CPT, HCPCS, modifiers, diagnosis and procedure coding, charge capture and documentation review — in specialty-specific workflows, not a generic template.
Claims are prepared, validated and scrubbed before they reach the payer, then submitted to the correct SMMC plan, PIP carrier, Florida Blue, workers’ comp carrier or MA payer in the format each expects. Rejections are monitored and corrected — not left in a queue.
A denial is more than an unpaid claim. It can reveal a problem with eligibility, coding, authorization, documentation, medical necessity, timely filing, payer information or coordination of benefits. We investigate the cause, manage corrections and appeals, and track recurring patterns by payer so the same denial stops repeating.
Outstanding A/R is revenue you have already earned. We organize and prioritize balances by aging, payer, claim status, amount and recovery opportunity, then work unpaid claims, underpayments, partial payments, stalled claims and appeals. A/R should be actively managed, not simply reported.
ERA/EOB and patient payments posted and reconciled against claims, with contractual adjustments, discrepancies, underpayments and unposted amounts surfaced — including PIP’s 80% reimbursements and MA adjustments tracked separately.
Patient responsibility keeps growing. Statements, reminders, online payment options and clear balance communication make the 20% PIP share and MA copays easier to collect while keeping the patient experience professional.
Authorization requirement checks, requests, documentation coordination, status tracking and payer follow-up across SMMC plans and MA carriers — so a missing auth doesn’t become a denied claim weeks later.
Provider enrollment, payer applications, CAQH, Medicare, Florida Medicaid and commercial enrollment, recredentialing, demographic updates and enrollment tracking — including the Trading Partner and EDI linkage Florida Medicaid requires, and renewal dates before they suspend your claims.
Florida Medicaid, PIP and Workers’ Compensation Billing
Three Florida payer systems with their own enrollment, authorization and reimbursement rules — and the three that generate the most avoidable denials.
Florida Medicaid (SMMC) Billing Support
Plan-and-region routing, plus Trading Partner linkage
Florida Medicaid has specific enrollment requirements — including that billing agents processing transactions on a provider’s behalf must be enrolled and linked to the provider group. BilNow organizes eligibility, claims, prior authorization, posting, denials, A/R and enrollment support around the member’s actual plan and region, and tracks agreement renewal dates before claims suspend.
PIP / No-Fault Billing
14-day rule, 80% reimbursement, separate fee schedule
BilNow screens for PIP eligibility at intake, tracks the 14-day initial-treatment requirement, bills the PIP carrier as primary under its fee schedule, and coordinates the patient’s 20% share.
Florida Workers’ Compensation Billing
Authorization rules and DFS reimbursement manuals
Florida law sets specific requirements for provider authorization to treat under workers’ compensation, with an exception for emergency care, and the Department of Financial Services publishes its own reimbursement manuals, billing forms and reporting requirements. BilNow supports claim submission, coding, authorization workflow, documentation review, follow-up, denials and A/R for workers’ comp.
Medicare Advantage
60%+ of Florida seniors, each plan its own rules
MA workflows are kept separate from Original Medicare and Medicaid so each plan’s prior-auth and appeal rules are followed and authorization gaps are caught before submission.
How to Choose a Medical Billing Company in Florida
Six questions that reveal whether a billing company actually knows Florida.
National vendors say yes to everything. These questions separate real Florida experience from a sales script.
QUESTION 1
Are you enrolled and linked as our Florida Medicaid billing agent?
Florida requires billing agents and clearinghouses to be enrolled and linked to the provider group, with an EDI agreement. Ask for proof — not a yes.
QUESTION 2
Which SMMC regions and plans have you billed?
If they can’t name Sunshine, Simply, Humana and Molina and how their rules differ, they haven’t billed Florida Medicaid.
QUESTION 3
How do you handle PIP and the 14-day rule?
Ask what happens when a patient shows up on day 15. A Florida-ready partner has a workflow for that.
QUESTION 4
Do you track our Medicaid agreement renewal dates?
The renewal link only appears in a 90-day window. Miss it and claims suspend with no retroactive payment.
QUESTION 5
Do you treat Florida Blue like other Blue plans?
The right answer is no — it’s an independent GuideWell licensee with its own portal, windows and workflows.
QUESTION 6
How is pricing structured, and who works my account?
Percentage of collections aligns incentives. Ask for the names and credentials of the people on your account.
BilNow vs. a Typical Billing Vendor
What Florida practices actually get — side by side.

|---|---|---|
| Florida payer rules | Routing by SMMC plan and region, PIP and workers’ comp workflows, Florida Blue handled on its own rules | One national template |

| Agreement renewals | Renewal dates tracked before claims suspend | Not monitored |
| Who works your account | A named team — billers, coders, an RCM lead, a billing manager | Rotating reps |
| Your EHR | You keep it; we work inside it | Migration to their system |
| Reporting | Clean claim rate, denial rate, A/R days, collection rate and patient payments, visible any time | A monthly PDF |
| Pricing | From 2.7% of collections — below the 2.95% starting rate some Florida vendors advertise — no setup fee, no long-term contract | Setup fees, minimums, per-claim add-ons |
Specialty-Specific Medical Billing in Florida
Your specialty deserves specialized billing.
A cardiology practice doesn’t bill like behavioral health. Pain management doesn’t operate like primary care. Dermatology doesn’t share a coding mix with orthopedics. And in Florida, your specialty decides how much PIP, MA or SMMC volume you carry.

Find out where your Florida revenue is going.
Maybe your current billing company is doing a good job. Maybe A/R is aging, one MCO keeps denying the same code, PIP claims are missing the 14-day window, or a Medicaid agreement is quietly approaching expiration. You shouldn’t have to guess.

Florida Medicare Contractor and Payer Checklist
Florida Medicare Part B claims are processed by a single contractor, and Florida practices also juggle Medicaid managed care plans and no-fault PIP claims. The Medicare facts below link to the CMS page they come from; payer-specific deadlines are tracked separately.
| Medicare MAC (Part A and B) | First Coast Service Options, Inc., A/B MAC Jurisdiction N (JN), which covers Florida, Puerto Rico and the U.S. Virgin Islands. Source: CMS, A/B MAC Jurisdiction N. |
|---|---|
| Jurisdiction scale | CMS reports Jurisdiction N serves roughly 2.4 million fee-for-service beneficiaries across 277 Medicare hospitals and handles about 8.1% of the national Part A and Part B claims workload. Source: CMS, A/B MAC Jurisdiction N. |
| Where to check other Florida deadlines | Florida Medicaid managed care plan windows, PIP and commercial prompt-pay timelines differ by payer. Look them up on our timely filing limits page and confirm with each plan’s provider manual. |
Because Florida Medicare claims all route through one contractor, a consistent First Coast submission workflow shortens the path from encounter to payment. Deadlines for other payers are tracked on our timely filing limits page. Last checked against the cited sources on 3 October 2026; always confirm current rules with the payer.

Frequently Asked Questions
What medical billing services does BilNow provide in Florida?
End-to-end medical billing and revenue cycle management: patient registration, eligibility and benefits verification, medical coding, claim scrubbing and submission, denial management, payment posting, patient billing, A/R recovery, prior authorization support, credentialing and reporting — all built around Florida’s payer mix.
Is BilNow enrolled as a Florida Medicaid billing agent?
Florida requires billing agents and clearinghouses that submit transactions on a provider’s behalf to be enrolled and linked to the provider group, with an EDI agreement in place. BilNow handles this linkage as part of onboarding — and we recommend asking any Florida billing vendor to demonstrate it before you sign.
Do you work with Florida Medicaid (SMMC) plans?
Yes. BilNow bills through Florida’s Statewide Medicaid Managed Care program — Sunshine Health, Simply Healthcare, Humana Healthy Horizons, Molina, Aetna Better Health and UnitedHealthcare Community Plan — routing by the member’s plan and region.
Can you handle Florida PIP / no-fault billing?
Yes. BilNow screens PIP eligibility at intake, tracks the 14-day initial-treatment requirement, bills the PIP carrier as primary under its fee schedule, and manages the patient’s 20% share.
Does BilNow handle Florida workers’ compensation billing?
Yes. Florida sets specific authorization requirements for treating under workers’ comp, with an exception for emergency care, and DFS publishes its own reimbursement manuals and billing forms. BilNow supports claim submission, coding, authorization workflow, documentation review, follow-up, denials and A/R.
Do you bill Florida Blue?
Yes. Florida Blue is an independent GuideWell licensee with its own provider manual, 365-day filing window and EDI workflows. BilNow works within Florida Blue’s rules rather than applying another Blue plan’s.
What happens if our Medicaid provider agreement expires?
Claims suspend until the renewed agreement is approved, and that period is generally not reimbursed retroactively. The renewal link only appears in your portal during the 90-day window before expiration, so BilNow tracks renewal dates as part of credentialing.
How do you bill snowbird patients with out-of-state insurance?
We verify the out-of-state plan at registration, confirm whether it has a Florida network, and route Blue plan members through BlueCard via Florida Blue. Out-of-state Medicare Advantage members are checked for network status before the visit so an out-of-network denial isn’t the first time anyone finds out.
Can BilNow work with our existing EHR?
Yes — including athenahealth, eClinicalWorks, AdvancedMD, Kareo (Tebra), NextGen and many others. The exact workflow is set during onboarding.
Does BilNow work with small medical practices?
Yes. BilNow supports solo providers, small and private practices, multi-provider groups, hospitals, labs and other healthcare organizations.
How much does medical billing cost in Florida?
Pricing depends on your specialty, provider count, claim volume, services required and revenue-cycle complexity. BilNow offers a transparent percentage-of-collections model starting at 2.7% for qualifying practices, with no setup fee.
Which Medicare Administrative Contractor processes Medicare claims in Florida?
First Coast Service Options, Inc. is the A/B MAC for Jurisdiction N, which covers Florida, Puerto Rico and the U.S. Virgin Islands, according to CMS. BilNow submits Florida Medicare Part B claims to First Coast and follows them through remittance.
How does BilNow price medical billing in Florida?
BilNow charges 2.7% of collections, with no setup fee and no long-term contract. The pricing page explains how it works.
Where is BilNow based, and does it serve Florida?
BilNow is headquartered at 430 Fifth Avenue, New York, NY 10018, and serves Florida practices, including those in Miami, Orlando, Tampa, Jacksonville and Fort Lauderdale.
Where can I check Florida payer filing deadlines?
Use the timely filing limits page for payer deadlines and the denial code lookup to decode a denial. BilNow also works denials through denial management.
