Top 10 Revenue Cycle Management Companies in the USA (2026)
Updated October 2026 · By the BilNow Editorial Team · Facts checked against each company’s own website and reputable news sources
Revenue cycle management (RCM) companies run the financial side of a medical practice: eligibility checks, coding, claim submission, payment posting, denial follow-up and collections. Some work as outsourced teams, some sell software, and some do both. This list is for practice owners, administrators and billing managers who are comparing revenue cycle management companies in the USA, and for anyone searching for RCM companies who wants a plain side-by-side view. The right choice depends on your size, specialty and EHR.
It covers ten companies, from billing firms built around physician practices to enterprise outsourcers and software vendors. Because the group is mixed, each entry says who the company suits best, including when it mainly serves hospitals. We used each company's own website and reputable news sources, checked in October 2026. Where a detail is not published, we say so. BilNow wrote this list and places itself first, so read that entry with that in mind.
How we ranked the companies
- Fit for physician practices: whether the company says it serves practices and physician groups, or mainly hospitals and health systems.
- Services covered: billing, coding, credentialing, eligibility verification, prior authorization, denial management and A/R follow-up.
- Pricing transparency: companies that publish a rate rank higher than those that quote on request.
- EHR coverage: whether the company works inside your existing EHR or ties its service to its own software.
- Contract terms: whether the company states a contract length, a setup fee or exit terms.
- Disclosure: BilNow wrote this list and ranks itself first, and these criteria reward published pricing and simple contracts. After BilNow, outsourcers that serve practices come before enterprise outsourcers and software-only vendors. We used public information only, and no company was asked to review its entry. Figures that come from a company are marked as company figures.

How to choose a revenue cycle management company
Ten companies, one question: which one fits your practice? Use these five questions to compare any RCM company on this list or off it. Ask every company the same questions, in writing, so the answers line up.
1. What does the fee cover? Ask what the percentage or rate applies to, and ask about setup, training, statement and reporting fees. On the pages we reviewed, BilNow is the only company on this list that publishes a rate, so request every other quote in writing and compare all of them on the same terms. Ask for a sample invoice so you can see how the fee is calculated.
2. Does it work in my EHR and my specialty? Ask for the exact name and version of your EHR on its supported list, and ask how many providers in your specialty it bills for. Some companies here sell their own software, so check whether you would need to change systems. Ask for a reference practice that uses the same EHR.
3. Which metrics will you report? Ask for first-pass acceptance, denial rate and days in A/R, and how each one is calculated. Ask for references from practices of your size and specialty, and speak to at least one. Ask how often reports arrive and who reviews them with you.
4. What are the contract terms? Ask about contract length, notice to leave, data ownership and what happens to open claims if you go. A short or flexible term lets you test the service before you rely on it. Ask whether payments go into an account you control.
5. Who will work on my account? Ask who your day-to-day contact is, whether staff are trained on HIPAA, and whether the company will sign a business associate agreement before it handles patient data. Ask how many accounts each biller handles.

Frequently asked questions
What does a revenue cycle management company do?
A revenue cycle management company handles the steps between a patient booking a visit and the practice being paid in full. That usually includes eligibility checks, prior authorization, coding, claim submission, payment posting, denial management, A/R follow-up and patient billing. Some companies take over the whole cycle, some handle only billing and collections, and some sell software for your own staff to use. Reporting on collections and denial trends is usually part of the service.
What is the difference between an RCM company and a medical billing company?
A medical billing company usually focuses on claims: submitting them, posting payments and following up on denials. An RCM company covers more of the path, including eligibility, coding, credentialing, collections and reporting. Many companies use both terms for similar services, so ask for a written list of what is included and what costs extra. Some also advise on front-desk workflow and payer enrollment. Neither term fixes a particular scope.
How much do RCM services cost?
Most outsourced RCM companies charge a percentage of collections. Others charge per claim, per provider or by the hour. BilNow publishes 2.7% of collections with no setup fee. AdvancedMD, a software vendor, says typical RCM services run about 4-8% of monthly collections. We found no published rate for the other companies here, so ask what the fee applies to and request a written quote. A percentage fee ties the company's income to yours, but the cost rises with collections.
Should a practice outsource RCM or keep it in-house?
It depends on size and volume. In-house billing gives you direct control, but it adds salary, benefits, software, training and cover for time off. Outsourcing turns much of that into a fee that moves with collections and brings a team with payer and denial experience. A contract with no long-term commitment lets you test an outside team before you rely on it. Some practices outsource everything, and some keep front-desk work in-house.
How do you switch RCM companies?
Start with the exit terms in your current contract. Sign a business associate agreement with the new company, give it access to your systems, and hand over open claims and balances. Update remittance and deposit details with payers. Ask for a written transition plan and keep the old company working on open claims until the new one is live. Allow several weeks, depending on payers. Check who owns your data and how to get a copy.
What metrics should I track with an RCM company?
Track first-pass claim acceptance, denial rate, days in accounts receivable, net collection rate and the share of A/R older than 90 days. Ask how each figure is calculated, because definitions differ between companies. BilNow, for example, reports 98% first-pass acceptance and a denial rate under 5%. Ask any company for the same figures and the method behind them. Review the numbers monthly and by payer, so one weak payer does not hide behind a good average.
